It is of a check in at the Lone Palm Hotel in Palm Springs, Ca.  It is of Phyllis checking in under an assumed name.  Probably one of her assignations.   She had many.  On the ticket she ok's the charges and fills out her real name and address on the reverse.  Note:  two guests...
